Study basis or key toxicological reasoning for E332
In a randomised double-blind trial, 57 adults received 30 to 60 mEq of potassium citrate daily or placebo for up to three years. Adverse reactions were described as mild, and only two potassium citrate recipients withdrew because of them [1]. A systematic review of seven randomised trials involving 477 adults found that upper gastrointestinal disturbance was the main adverse effect of citrate therapy, although discontinuation due to adverse events was more frequent than with control treatment [2]. In a clinical cohort of 198 children receiving potassium citrate with a ketogenic diet, no adverse effects were reported [3]. A 2026 analysis of 408 spontaneous reports found that most signals involved formulation, swallowing or gastrointestinal issues rather than unexpected systemic toxicity, but such reports cannot establish incidence or causality [4]. No modern additive-specific animal NOAEL or LOAEL was identified. Dedicated long-term cancer and reproductive studies are limited, while available human exposure evidence is based mainly on medicinal use at doses well above typical food-additive intake.

- No expected effect at normal food levels. The quantities used to regulate acidity are generally much lower than therapeutic potassium citrate doses.
- Gastrointestinal discomfort at higher doses. Medicinal use can cause nausea, abdominal discomfort, diarrhoea or heartburn, especially with concentrated products [1,2].
- Excess blood potassium in susceptible people. High supplemental intake can contribute to hyperkalaemia when kidney function is impaired or potassium-retaining medicines are used.
- Swallowing or throat irritation from tablets. These effects mainly concern large extended-release medicinal tablets, not E332 dissolved in food [4].
- Rare skin reactions. Rash has been reported infrequently during medicinal use, but a consistent allergy pattern has not been established [2,4].
Should You Avoid E332 – Potassium citrates food additive?
Most people do not need to avoid E332 in food. Its permitted technological use contributes far less potassium than prescription therapy or concentrated supplements. People with chronic kidney disease, known hyperkalaemia, adrenal disorders or medicines that reduce potassium excretion should be cautious with potassium-containing supplements and salt substitutes. What is E332 – Potassium citrates food additive?
E332 is the collective E-number for two potassium salts of citric acid. E332(i) is monopotassium citrate, also called potassium dihydrogen citrate. E332(ii) is tripotassium citrate, the form commonly shortened to potassium citrate. Both are white, water-soluble crystalline materials made by reacting citric acid with potassium hydroxide or potassium carbonate under controlled conditions.
The two forms differ in how many acidic hydrogen atoms of citric acid have been replaced by potassium. Monopotassium citrate is more acidic, while tripotassium citrate provides stronger alkalising and buffering effects. Food manufacturers select the form and amount needed to adjust pH, stabilise flavour, control mineral interactions or improve texture.
When dissolved, E332 separates into potassium and citrate ions. Potassium is an essential electrolyte, while citrate participates in normal energy metabolism and can bind certain metal ions. This familiar biological handling supports the low-risk assessment at ordinary food-use levels.
